Which version of sequenceserver are you trying to run? (sequenceserver -v)
Are you able to run sequenceserver on the command line first? That is without using Phusion Passenger.
Do you have a Gemfile.lock file in the directory where SequenceServer is installed? Can you try if Phusion Passenger works after deleting that file.
